The selected HCC will deliver and oversee the day-to-day clinical operations of our community. Responsibilities include: Managing the health, functional and psychosocial status of residents, initiate individualized service plans and on-going assessments, perform on-site assessments of residents admitted, and conduct pre-admission screening of prospective new residents. You will provide training, supervision and monitor staff in the administration of medications, the maintenance of resident charts, review documentation performed by care giving staff, and meet with family members on a regular basis regarding their loved ones care.
