The RN Case Manager is responsible for assisting with the development, organization, implementation, monitoring and evaluation of activities related to the outcome management of assigned patients. Acts as a resource, consultant, and educator to all staff in the planning and delivery of patient care within the Network.

DUTIES AND RESPONSIBILITIES:

' Uphold and support the philosophy, goals, and objectives of the Medical Management department at Healthfirst to provide quality, cost-effective care to all members.
' Assume a leadership role in the multi-disciplinary care team in order to insure high quality patient care and appropriate allocation of member services and resources.
' Conduct educational programs for all members of the Healthfirst team, which support the continued growth of activities that are focused on measurable patient outcomes.
' Maintain and develop professional competence via participation in professional organizations and continuing education programs.
' Manage patient care with Medicare members, their families or significant others and coordinate with the PCP, SCP and all members of the health care team facilitating the patient's care across continuums of care episodes.
' Perform preadmission, concurrent, and retrospective reviews to evaluate appropriateness of admission, need for continued stay, length of stay, utilization of resources, patient outcomes, and usage of other services post encounter.
' Recommend changes in-patient care practices.
' Identify opportunities for and facilitate member transfer to: a) hospital of enrollment/other appropriate in-network hospital when hospitalization occurs out-of-network; or b) hospital of enrollment when hospitalization occurs at another network hospital.
' Evaluate and pre-authorize request for inpatient/outpatient, specialty care, home care, DME, and transportation services from network and out-of-network providers in accordance with departmental guidelines.
' Collect and evaluate data and statistics based on identified indicators and recommend changes in-patient care practices.
' Participate in research and quality monitors in order to improve practice patterns.
' Function as a clinical resource person/educator for the multi-disciplinary health care team on an ongoing basis in order to maximize quality of patient care, effectively manage length of stay and utilization of resources.
' Maintain open lines of communication with an emphasis on supportive collegial relationships.
' Act as a resource/educator/liaison to all levels of staff both internally at Healthfirst and in the network regarding Healthfirst medical management policies and procedures.
' Coordinate resources allocation which maximizes efficiency within the network while maintaining cost containment.
' Assist in the development of patient care standards.
' Assist with identifying opportunities for and facilitating alternative care options based on member needs and assessments.
' Perform other related duties as required.
' Assess all inpatient, emergency and ambulatory care for appropriateness of setting and services according to preestablished criteria and guidelines.
' Assess member's physical, psychosocial and discharge planning needs through communication with appropriate hospital staff including attending practitioner, utilization review staff, discharge planners, social workers, and coordinates care accordingly.
' Contribute to the development of policies and procedures.
' Responsible for compliance, in practice, with regulatory mandates.
' Contribute to the development of patient focused of care and initiates the process as per established guidelines.
' Document all interventions and all telephone encounters with providers, members, and vendors in the appropriate system in accordance with established documentation standards to insure integrity of member services provided over the continuum and over time.
' Document and close all authorizations and make a determination as to whether the requested services should be paid or referred for possible denial to insure expeditious claims processes.

SKILL SETS:

' Effective verbal communication skills, appropriate, professional conversational telephone manner
' Ability to apply nursing process and systems theory to problem solving activities
' Familiar with current relevant medical standards of practice
' Knowledge of concepts and philosophy of Case Management and regulations of utilization review
' Good writing and excellent telephone assessment skills
' Ability to serve as a liaison and educator, and coordinate resources on a continued basis
' Ability to assess appropriateness of inpatient care

EXPERIENCE:

Minimum of 2 years clinical experience, or previous Case Management, Utilization Management experience dealing with Medicare populations.

EDUCATION:

Graduate of an accredited nursing program with a RN certificate
Baccalaureate degree preferred

OTHER REQUIREMENTS:

Licensed to practice as a registered nurse in the state of New York with current registration.

Evidence of continuous education in patient care management and case management. Knowledge of concepts and philosophy of Case Management and regulations of utilization review required
Experience in a managed care environment.

* Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities.

* Monitors the safety and effectiveness of the environment and equipment.

* Participates in activities to facilitate good interpersonal communications with the chosen facility.

* Supports professional practice by observing the following: Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) JCAHO, state and local laws; 5) Patient's Bill of Rights; and 6) Maxim Healthcare, nursing and client hospital policies.

* Other duties as assigned


Minimum Requirements:

* Current Registered Nurse License for the state in which the nurse practices.

* Full command, verbal and written, of the English language.

* Current Health Certificate (per facility contract or state regulation).

* Current PPD or Chest X-Ray.

* One year prior professional nursing experience preferred.

* Current BLS card.

Maxim Staffing is searching for a qualified Registered Nurse to work in an elite facility in Arlington. This is a full time position working 7P - 7A. Two years of experience in an Oncology unit is required.

Evaluates, plans, implements, and documents nursing care for oncology patient population.

Responsibilities include:

* Provides specialized oncology nursing care to patients in a hospital or clinic setting.

* Performs nursing procedures and prescribed medical treatments.

* Observes and assesses chemotherapy patients' conditions.

* Assures patient safety and stability; reports significant changes to supervisor or physician; records nursing notes on patient chart.

* Interviews and observes patients to as certain medical history and current condition; reviews patients' records; takes vital signs, assesses current condition and pain level.

* Administers chemotherapy, blood products, IV fluids and medications.

* Validates chemotherapy doses for accuracy.

* Reviews and carries out physician's orders.

* Reports progress and unusual occurrences.

* Identifies patient needs and healthcare problems; develops goals, service delivery protocols, and nursing plan of care; evaluates patient progress and modifies care plans as needed.

* Records and reports all information within scope of authority.

* Performs assessment of patients; evaluates symptoms, and determines essential care required.

* Prepares patients for physician's examination, and assists in examination and treatment of patient.

* Educates patients and families about cancer diagnosis, prognosis and treatment options.

* Instructs individual patients and family on follow-up care, toxicity and side effects of chemotherapy and radiation therapy.

* Assists in assessment of pain management.

* Other duties as assigned


Qualifications include:

* Must be able to communicate effectively.

* Current RN license.

* Prefer at least 1 year of experience in oncology.

* Current BLS card.

* Current physical exam per state or contract requirements.

* Successful completion of Maxim screening tests with a score of 80% or greater.

* Must pass criminal background screening.

* Current TB test or chest x-ray.

* PALS/ACLS if per contract requirements.

Qualifications include:

* Current Registered Nurse License for the state in which the nurse practices.

* Current Health Certificate (if applicable).

* Current PPD or Chest X-Ray.

* The physical ability and perceptual acuity, (e.g., visual, hearing) to satisfactorily perform all direct and indirect patient care functions.

* Full command, verbal and written, of the English language.

* Demonstrate good interpersonal communication skills.

* Demonstrate the ability to effectively use the nursing process.

* Minimum of one year acute care experience in the area of expertise within the last three years preferred.

* Current BLS card; current ACLS/PALS card (per facility requirement).

* Successful completion of the Maxim screening for the clinical specialty area(s) in which the RN practices with a score of 80% or greater.

Registered Nurse - RN - Emergency Room Nurse
Currently seeking an experienced and energetic Emergency Room Registered Nurse to work at an elite medical facility in Houston. Flexible scheduling is available working full time or part time.

Skilled Registered Nurse / RN is needed to work in local facility to provide direct patient care. Registered Nurse / RN will observe, assess, and record symptoms, reactions, and progress in patients; assist physicians during surgeries, treatments, and examinations; administer medications; and assist in rehabilitation.

Registered Nurse / RN will also develop and manage nursing care plans, instruct patients and their families in proper care, and help individuals and groups take steps to improve or maintain their health.

Qualified nurse should be an advocate and health educator for patients, families and communities.

The Registered Nurse, as a professional practitioner employed by Maxim Healthcare Services, Inc., assumes responsibility and accountability for the application of the nursing process and the delivery of patient care. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision. The nurse demonstrates critical thinking and performance ability in the coordination of patient care. Consistently performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.

Essential Duties and Responsibilities:

* Utilizes the nursing process to assess, plan, implement and evaluate patient care.

* Performs focused interview to identify specific patients needs.

* Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.

* Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

* Documents the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.

* Identifies and prioritizes actions based upon patient care requirements.

* Monitors consistency between patient plan of care and actions of medical staff.

* Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard.

* Provides a safe, comfortable and therapeutic patient environment.

* Initiates and evaluates patient and/or family education.

* Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.

* Documents patient's response to nursing and other interventions.

* Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities.

* Monitors the safety and effectiveness of the environment and equipment.

* Participates in activities to facilitate good interpersonal communications with the chosen facility.

* Supports professional practice by observing the following: Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) JCAHO, state and local laws; 5) Patient's Bill of Rights; and 6) Maxim Healthcare, nursing and client hospital policies.

* Other duties as assigned


Minimum Requirements:

* Current Registered Nurse License for the state in which the nurse practices.

* Full command, verbal and written, of the English language.

* Current Health Certificate (per facility contract or state regulation).

* Current PPD or Chest X-Ray.

* One year prior professional nursing experience preferred.

* Current BLS card.

* Successful completion of screening tests for the clinical specialty area(s) in which the RN practices with a score of 80% or greater.

* Maxim background screening


Physical/Work Environment Requirements:

* May work at off-site facility/ client site

* May be exposed to blood-borne pathogens, chemotherapeutic and toxic chemicals
