The hospice RN is essentially a complete case manager. That is, the RN is responsible for the assessment of the physical, emotional, spiritual and social needs of the patient as they relate to terminal care, the implementation of interventions established by the team to address those needs and the evaluation of the effectiveness of those interventions. The RN works within a team comprised of the patient and family, their attending physician, the hospice physician, a social worker, a chaplain and other disciplines as determined necessary by this core team. The RN's 'office' is the patient's home setting - whether that's a private residence, a nursing home, an assisted living or personal care home. All of the team members function together to meet the needs of the patient but the RN has the primary responsibility for driving the care plan - hence, the title 'case manager.'

Here at VistaCare we provide a comprehensive orientation program to introduce the field of hospice nursing to all nurses. Being a hospice nurse will challenge you in many ways - some of them fundamental. It is like nothing else you've ever done.

A day in the life of a hospice nurse includes visiting patients, making phone calls to patients, care givers, physicians, documentation, collaboration with other team members, participating in team meetings or other program designated tasks.





Requirements
Registered Nurse with current state license. Minimum of one year recent medical, surgical, oncology or home care experience. Complies with acceptable professional standards and practice. Demonstrates good communications, negotiation and public relations skills. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. Reliable transportation, current driver's license and current auto insurance required.
