RN Case Manager
About the Job
EmblemHealth, the parent company of HIP Health Plan of New York and GHI-Group Health Incorporated, is seeking an RN Case Manager.
Essential Duties:
Achieves optimal benefit and results from the comprehensive management of patients with chronic and/or catastrophic illness, patients who are frail elderly and/or patients whose illness is complicated by challenging psychosocial conditions.
- Identifies patients appropriate for case management early in their disease process, preferably at enrollment with HIP Health Plan of New York.
- Develops, facilitates, and communicates a plan of care in partnership with the patient, his/her significant other, the primary and attending physicians, and various providers.
- Optimizes both the quality of care and the quality of life for the patient.
- Minimizes risk for the patient, the employer and the Plan.

Qualifications:
· Graduate of an accredited 2 or 4 year nursing program (advanced degree preferred).
· Current unencumbered professional New York State RN License.
· Certification as a Case Manager preferred.
· Minimum of 5 years of clinical experience in acute high-tech or home health setting.
· Prior case management and or managed care experience preferred.
