Large Healthcare Agency seeking REGISTERED NURSE Case Managers
Responsibilities:
* Provide telephonic case management of long-term health care services allowing plan members to remain in the community by collaborating with health care providers and developing personalized plans of care.
* Coordinate, authorize and track patient care for home care cases focusing on individuals who wish to remain at home.
* Work in conjunction with member service representative.
* Coordinate caseload for the 70 patients.
* Provide Case management including coordinating, authorization and tracking of patient care with accordance to the plan of care.
* Decide and provide professional recommendation based on the patient's changing needs in collaborate with the patient, family and physician.
* Coordinate discharge planning of hospital admission by identifying patient who may have complicated discharge needs and require ongoing treatment in an may have complicated discharge needs and requires ongoing treatment in and alternative setting.
Requirements:
* RN – REGISTERED NURSE IN THE STATE OF NY
* BSN or relevant Bachelor’s degree preferred
* 2 years experience in Case Management and/or Home Care preferred
* Computer literacy a must
$76K-$78K
