Position:

* The Hospice RN is responsible for the planning, direction, coordination, provision and documentation of care for terminally ill patients from day of admission through day of discharge.
* Works in compliance with federal and state regulations, reimbursement guidelines as well as the organization’s policies and procedures.
* Assures quality care by reviewing and assessing the multidisciplinary team including RN, Social Worker, C.N.A., Chaplain, Bereavement Coordinator, and Volunteers, in all areas to facilitate effective, efficient patient care and assure quality care.
* The Hospice Nurse maintains and participates in quality control programs.
* RN reports to the Patient Care Manager.

Education and Experience:

* A Registered Nurse from an accredited program with current state licensure
* One (1) year nursing experience in as an RN in hospice, home care or community health care.
* Valid Drivers License, proof of automobile insurance, reliable vehicle.
