Job Details:
This is for the GI clinic. Two 10-hour shifts/week.
JOB SUMMARY:
Care Coordinator responsibilities include coordinating patient assessment, planning of care, discharge planning; and administering patient care and patient/family education. Provides necessary intervention, treatment, documentation, and professional clinical team development. Performs additional team functions as required to maintain effective customer services.
MINIMUM QUALIFICATIONS:
Graduate of accredited school of Nursing with current California Registered Nursing Licensure
Two years pediatric nursing experience
At least one Care Coordinator in each Care Team must have BSN required to meet California Children Services standards.
