We are looking for RNs and LPNs to join our team!

As a Registered Nurse you will care for assigned patients by assessing needs, implementing nursing care plans, providing appropriate interventions, making appropriate revisions, and evaluating outcomes of care.

RN:

· Participates in obtaining and documenting admission and assessment data and development of initial plan of care.
· Delivers nursing care to patients according to agency policies and procedures. This includes teaching, providing direct care, counseling, case management, and monitoring.
· Functions independently in the home setting according to established productivity standards.
· Documents findings accurately, in a timely manner and on appropriate forms according to agency policies and procedures.
· Instructs Home Health Aides on specific patient care procedures within approved policies and procedures.
· Clinically evaluates LPNs and CHHAs during home visit, as requested.

REQUIREMENTS
To qualify you must have:

· Must have a current LA RN license.
· Minimum one year of experience in nursing.

LPN
Provides and documents professional nursing care within scope of practice standards under supervision of RN for assigned patients. Maintains a safe, comfortable and confidential environment for the patient. Participates as a team member to ensure appropriate resources to all internal/external customers. High school graduate or equivalent required. Post high school specialized vocational training preferred. 12 months as LPN experience required. 1-2 years LPN in home healthcare experience preferred. LA LPN license and valid drivers license required.
