Job Responsibilities Include:
• Obtains medical history, particularly as it relates to the present condition, from the patient and/or family member.
• Provides skilled nursing care as outlined in the nursing care plan to include, but not be limited to the following:
• Coordinates and supervises the care given by the LPN, Home Health Aide and/or Personal Care Attendant assigned to provide care to the patient.
• Charts those services the Visiting Nurse – RN has rendered to the patient, notes changes in the patient’ condition and/or family and home situation, makes revisions in the nursing care plan as needed, records supervisory visits, evaluates patient care and progress and closes charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.
• Maintains confidentiality of client’s situation, records and services provided according to HIPPA regulations
• Other duties supporting direct patient care
All clinical positions require state licensure/certification, CPR Certification and valid driver’s license.

Requirements:
• State licensure and certification
• Physical within 6 months prior to first patient contact
• Current TB results (within last 12 months)
• Current CPR Certification
• Must have valid driver’s license
