MDS Nurse - RN: Long-Term Care

Albuquerque, New Mexico

We have immediate openings for an MDS Nurse at our Canyon Transitional and Rehabilitation Center and our St. John Healthcare and Rehabilitation Center in Albuquerque, NM.

The successful candidate will possess the following:

* Current RN in good standing and currently licensed by the state of New Mexico

* Minimum of one year of long-term care MDS experience

* Knowledge of PPS reimbursement system

* Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care

* Previous experience in MDS and Care Plan completion preferred

* Computer skills and Windows applications experience preferred

Responsibilities will include:

* Direct the facility interdisciplinary process, computer encoding, and electronic State transmission of the MDS

* Complete nursing-assigned MDS items and Care Plans

* Schedule and facilitate Care Plan Meetings

The ideal candidate will be accurate, concise, detail-oriented and able to work cooperatively as a member of a team. You will also have the ability to organize, prioritize, and communicate effectively with residents, their families, and with all levels of the organization.
